Your Completed Application Checklist
Students in a NSW Government School should have the following documents to send in with their completed application :
- Completed Single Course Provision Application

- Include category for enrolment and suporting material
- All pages completed
- Signed by Principal, Supervisor, Parent/Guardian and Student
- Print out of Ghosted ERN form
- Payment either by cheque or credit card details: enrolment fee and course fees if applicable
- Languages Declaration for Stage 6 students (if applicable)
Students in a Non Government School should have the following documents to send in with their completed application :
- Completed Single Course Provision Application
