Your Completed Application Checklist

Your Completed Application Checklist

Students in a NSW Government School should have the following documents to send in with their completed application :

  • Completed Single Course Provision Application
    • Include category for enrolment and suporting material
    • All pages completed
    • Signed by Principal, Supervisor, Parent/Guardian and Student
  • Print out of Ghosted ERN form
  • Payment either by cheque or credit card details: enrolment fee and course fees if applicable
  • Languages Declaration for Stage 6 students (if applicable)

Students in a Non Government School should have the following documents to send in with their completed application :

  • Completed Single Course Provision Application
    • Include category for enrolment and suporting material
    • All pages completed
    • Signed by Principal, Supervisor, Parent/Guardian and Student
  • Payment either by cheque or credit card details: enrolment fee and course fees if applicable
  • Languages Declaration for Stage 6 students (if applicable)
