Enrolment Application Forms

The following three (3) forms will need to be completed when enrolling at Karabar Distance Education Centre.

NOTE: Some parts of the forms require signatures. The forms will need to be saved, printed and signed. They will then need to be uploaded, emailed, faxed or mailed to Karabar Distance Education Centre.

Choose the correct category (below) for your circumstance. Save, print, complete and sign the form using a blue or black pen.
