Your Completed Application Checklist

Students in a NSW Government School should have the following documents to send in with their completed application:

  • Completed Single Course Provision Application form 

         -- Include category for enrolment and supporting material 
         -- All pages completed 
         -- Signed by Principal, Supervisor, Parent/Guardian and Student     

  • Languages Declaration for Stage 6 students (if applicable)  
  • Exceptional Circumstances Application form (if applicable)


Note: An inter school journal will be created by Karabar Distance Education Centre if the enrolment application is approved. 

Students in a Non Government School should have the following documents to send in with their completed application:

  • Completed Single Course Provision Application form 

         -- Include category for enrolment and supporting material 
         -- All pages completed 
         -- Signed by Principal, Supervisor, Parent/Guardian and Student   
  • Languages Declaration for Stage 6 students (if applicable)
  • Exceptional Circumstances Application form (if applicable)

Note: A tax invoice will be created by Karabar Distance Education Centre if the enrolment application is approved.
