Your Completed Application Checklist

Students in a NSW Government School should have the following documents to send in with their completed application:

  • Completed Single Course Provision Application  

         -- Include category for enrolment and supporting material
         -- All pages completed
         -- Signed by Principal, Supervisor, Parent/Guardian and Student   

  • Payment either by cheque, EFT or online payment (or request an invoice) of enrolment fees and course fees if applicable 

  • Languages Declaration for Stage 6 students (if applicable) 


Students in a Non Government School should have the following documents to send in with their completed application:

  • Completed Single Course Provision Application 

         -- Include category for enrolment and supporting material
         -- All pages completed
         -- Signed by Principal, Supervisor, Parent/Guardian and Student  

  • Payment either by cheque, EFT or online payment (or request an invoice) of enrolment fees and course fees if applicable 

  • Languages Declaration for Stage 6 students (if applicable)
